    Colorectal Foreign Body: A Case Report

    Background:Emergency surgeons seldom encounter cases of foreign body ingestion/insertion. Both children and adults may present with ingestion or insertion of foreign body inside body cavitiesCase presentation: Two foreign nationals were brought by customs officer to emergency department with alleged history of insertion of cocaine packets through anus.Any subjects that caused omission from patients were admitted and rectal examinations were carried out. Rectum was filled with cocaine capsules which were retrieved manually as far as we could reach.Conclusion: It is likely that the use of various objects for anal eroticism is increasing, resulting in an increased incidence of retained rectal foreign bodies.Copyright©2012 Department of Forensic Medicine and Toxicology.     Evaluation of shoulder kinesthesia in patients with unilateral frozen shoulder

    Background: Intact sensory-motor system provides kinesthesia for maintaining joint stability. Joint proprioception or joint kinesthetic sensation is often used alternately to describe a deep sensation of joint motion and joint position sense. Mechanoreceptors located in superficial layer of joint capsule, muscle spindle, golgi tendon organ complex and ligaments around joint provides joint kinesthetic sensation. Frozen shoulder or adhesive capsulitis is one of the common musculoskeletal disorders encountered in Indian population with a prevalence of almost 50% older patients with diabetes and 2-10% in non-diabetic patients. This study assessed joint kinesthetic sensation during shoulder movements in people with frozen shoulder and healthy controls which may be affected due to presence of disease process or pain. Authors hypothesized that pain and alterations in non-contractile tissues (mainly capsule) around shoulder joint due to frozen shoulder may interfere with the joint kinesthetic sensation.Methods: Case group consisted of 41 participants with frozen shoulder and 41 healthy participants without shoulder pathology. Joint kinesthesia was assessed in frozen shoulders and dominant shoulders using angle reproduction test by actively reproducing the target angle placed passively by the therapist during flexion, abduction, medial and lateral rotation movements. The angle reproduced was measured using universal goniometer by the same therapist and 3 readings were recorded.Results: Results indicate significant differences between the mean difference of angle reproduction test during shoulder movements in cases of frozen shoulder and healthy group (p value <0.05).Conclusions: Significant shoulder kinesthetic deficits were observed in patients with unilateral frozen shoulder. Therefore, clinicians should consider rehabilitation of kinesthesia in treatment of patients with frozen shoulder

    Management and functional outcome of Galeazzi fracture dislocation: a single centre retrospective study

    Background:Galeazzi fracture dislocation is an unstable fracture dislocation of forearm that includes fracture of distal third or fourth shaft of radius with dislocation of distal radioulnar joint. The aim of the study was to analyse the management and functional outcome of Galeazzi fracture dislocation managed with fixation of radius fracture and distal radioulnar joint stabilisation with two Kirschner wires.Methods:Thirty one patients with Galeazzi fracture dislocation were managed with plate fixation for radius fracture and distal radioulnar joint stabilisation with 2 Kirschner wires. They were clinically and radiographically assessed for functional outcome as well as union, distal radioulnar joint stability and any arthrosis of the wrist joint.Results:Functional outcome was assessed based on disability of shoulder, arm, and hand index. 94% of the patients in our series had good to fair outcome with 6% having poor outcome.Conclusions:All Galeazzi fracture dislocation should be managed with plate fixation for radius fracture and Kirschner wire stabilisation of distal radioulnar joint followed by splinting for 6 weeks for best functional outcome

    Effects of Occupational Exposure on the Health of Rag Pickers Due to Fungal Contamination at Waste Dumping Sites in Gwalior (India)

    We investigated fungal contamination near different waste dumping sites and assessed the health risk factors of rag pickers associated with collection of waste in Gwalior during the year 2014-15. Petri plates were exposed at waste dumping sites and were transferred to the laboratory, analysis and identification was mainly carried out by culturing the fungal colonies by following standard procedures. A pretested questionnaire was used to evaluate the health problems among the rag pickers. Results indicated that all the dumping sites are contaminated with different types of fungal pathogens like Alternaria alternate, Aspergillus flavus, A. fumigates, A. niger, Cladosporium, Fusarium, Mucor, Penicillium and Rhizopus. Our study reported higher incidence of musculoskeletal and respiratory diseases among rag pickers. There is also strong need for carrying out similar assessment studies for other cities too. This will entail generation of more precise site specific information regarding fungal species and associated health risk factor

    VIBRATION ANALYSIS OF DRILLING OPERATION

    Vibrations are produced during any machining process. For drilling operation, analysis of these vibrations plays an important role in order to predict phenomenon of ‘chatter’. This paper emphasizes the analysis of vibration during drilling operation. The output results of analysis are useful to find out amplitude of vibrations produced with respect to drill size and spindle speed for standard rate of recommended feed/min. Analysis is quantified and tabulated as per available machining parameters of ‘THAKUR PELTER DRILLING MACHINE’ which is present in the Workshop of Fr. Conceiceo Rodrigues Institute of Technology, Vashi, Navi Mumbai. The optimum values of spindle speed and feed for maximium amplitude of transverse vibrations with respect to drill size are highlighted in the table and brought to the notice of Workshop Superintendent. The table of formulated results is displayed near this drilling machine

    Climate change adaptation and mitigation strategies for sustainable crop production

    Climate change is the main environmental problem facing humanity. Evidences over the past few decades show that significant changes in climate are taking place all over the world as a result of enhanced human activities through deforestation emission of various greenhouse gases and indiscriminate use of fossil fuels. Carbon dioxide (CO2) Methane and Nitrous Oxide are the major greenhouse gases. CO2 enters the atmosphere mostly through burning fossil fuels. Methane emissions also result from livestock and other agricultural practices and Nitrous Oxide is emitted during agricultural and industrial activities. Global atmospheric concentration of CO2 has increased from pre-industrial level of 280 parts per million (ppm) to 408 ppm in Feb 2018. Global projections indicate higher temperature of 1.5 to 4.5°C by the year 2050 as a result of enhanced greenhouse gases. There is medium confidence in that the Indian summer monsoon circulation weakens but this is compensated by increased atmospheric moisture content leading to more rainfall. There is medium confidence in an increase of Indian summer monsoon rainfall and its extremes throughout the 21st century under all RCP (Representative Concentration Pathways) scenarios

    A study of the effects of large dose of parenteral vitamin D (D3) on insulin resistance in type 2 DM patients

    Background: Over the past decade, vitamin D is more known as a hormone because of its extra - skeletal outcomes in various disease conditions, including diabetes. Most cells, including the pancreatic β-cells, contain the vitamin D receptor and they also have the capability to produce the biologically active 1,25-dihydroxyvitamin D [1,25(OH)2D3] which allows intracrine and paracrine functions. In vitro studies have shown that the active vitamin D metabolite 1,25(OH)2D stimulated insulin release by the pancreatic β-cells. Vitamin D is known to have immune modulatory and anti-inflammatory effects and reduces peripheral insulin resistance by altering low-grade chronic inflammation. This study was done to assess whether supplementation of vitamin D in type 2 diabetes mellitus (T2DM) patients with Vitamin D deficiency has any favourable effect on insulin resistance.Methods: It was a short term interventional study conducted at ASCOMS hospital Jammu including a total of 50 vitamin D deficient [25(OH) D 7.0%). All the 50 study participants completed the study and there were no changes either in anti-hyperglycemic drugs (including insulin) or antihypertensive drugs being used. After supplementation with a single high dose (600000 IU) of parenteral vitamin D3 changes in HOMA-IR (Homeostasis model assessment insulin resistance) were seen on follow up at 3 months.Results: Vitamin D3 supplementation improved insulin sensitivity, HOMA-IR decreased from 4.05±1.42 to 3.93±1.28 (p =0.011). It decreased equally in males (3.85±1.43 to 3.76±1.30) (p value=0.023) and females (4.24±1.42 to 4.10±1.27) (p value=0.021). HOMA-IR showed negative association with Vitamin D levels both at baseline and after 3 months of follow up.Conclusions: This improvement in insulin sensitivity is evidenced in our study by decrease in fasting insulin levels (FIL) and improvement in fasting blood sugars (FBS). It is due to both direct and indirect effects of Vitamin D3 on both insulin sensitivity and secretion

    Hepatoid carcinoma of ovary: a very rare histopathological finding

    Hepatoid carcinoma of the ovary is a rare histopathological variant that has phenotypic properties in common with hepato-cellular carcinoma and composed mainly of epitheloid cells. It presents with unilateral or bilateral ovarian masses with elevated serum α-feta-protein especially in post-menopausal female. We hereby discuss a very rare case of 58 years old female presented with abdominal pain and distension with high serum levels of CA 125 and AFP. She was diagnosed as hepatocellular carcinoma. CT guided FNAC of omentum revealed poorly differentiated carcinoma and on immuno-histochemical staining cells was positive for CK7, CK8/17 and AFP. The tumor cells were negative for CMA, PAX2/PAX8, CK20 and CK19. She was diagnosed with bilateral hepatoid carcinoma of ovary. This shows that histopathological examination and tissue immuno-histochemical staining plays an important role in distinguishing hepatoid variety of ovarian carcinoma from other ovarian tumors and hepatocellular carcinoma
